Learning objectives:

  1. Describe comprehensive process for community engagement in planning phase of community health improvement initiatives
  2. Identify multi-sector partners to address social determinants of health through community-clinic collaborations
  3. Design a care coordination system in partnership with community members and agencies to screen, refer, navigate and connect patients to address social needs and improve maternal and child health outcomes

Bio:
Robin Lankton is the Director of Population Health with UW Health. At the University of Wisconsin – Madison Robin holds faculty appointments in the Master of Public Health Program and Preventive Medicine Residency Program.  She is also the President of the Wisconsin Public Health Association and serves on the Board of Health for Madison and Dane County.  Robin brings more than 15 years of public health practice in health care, local public health department, managed care and non-profit sectors. Her areas of interest include collaborative partnerships, systems change, and supporting the conditions for all people to have the best opportunity to be healthy and thrive.
